How to Submit Your Printed Form

Download the Correct Form
Click "Download PDF" above and save the form to your device. Make sure you choose the right form — BS Hons or Diploma.

Print on A4 Paper
Print the form on plain white A4 paper. Do not scale or resize — print at 100% size to preserve all field dimensions.

Fill in Blue or Black Ink
Complete all required fields clearly using a blue or black ballpoint pen. Avoid corrections — use a fresh printout if needed.

Print on A4 Paper
Attach attested copies of your CNIC, DMCs, domicile certificate, passport photo, and any other required documents.

Submit to Admissions Office
Bring your completed form and documents in person to the IMI Admissions Office, Umarzai Bazar, Tangi Road, Charsadda.

Collect Your Receipt
The admissions officer will issue you a receipt with your application reference number. Keep this safe; you'll need it for follow-ups
